Botulinum Toxin

What is it?

Neurotoxin injections (Botulinum Toxin) are used in aesthetic medicine to soften or take away frown lines and wrinkles.  They work by blocking the transmission of nerve impulses to the muscle, resulting in a decrease in muscle action which lasts for 3-6 months. Apart from the immediate improvement in wrinkles, regular neurotoxin injections help to prevent  deep lines and wrinkles from forming and becoming ingrained.

Muscle relaxing injections are the most popular treatments around the world and the feel-good factor is unbelievable. Studies have shown that reducing the frown lines with injections is as effective as an anti-depressant in lifting one’s mood.

Most patient start this treatment in their late 20’s – 30’s.

Botulinum Toxin is also widely used to treat other medical conditions such as hyperhidrosis, cerebral palsy, overactive bladders, migraines, teeth grinding & clenching and strabismus (squinting).

What does a treatment entail?

The area to be injected will be cleaned and markings may be made on the skin.  Ice will be applied to the area to help numb it.  Small amounts (units) of Botulinum Toxin are injected into the area being treated in specific locations determined by your doctor.  Afterwards, there may be small, raised bumps which usually disappear within 15 minutes. Although we use a very small needle, bruising may occur.

Is there pain and downtime?

Ice is applied to the area being treated which helps make the injections less painful.  Most people tolerate the procedure easily.  If you are particularly scared of needles, please ensure that you arrive at least 30 minutes before your appointment and request topical anaesthetic cream to be applied.

When can I expect to see results?

Reduction in muscle action usually starts within 3 days with the maximal effect reached at 14 days.  Because Botulinum Toxin sometimes affects some muscles more quickly than others, we usually see our patients for a follow up on day 14 to check that results are optimal.

How many treatments do I need and how often?

The wrinkle-reducing effects of Botulinum Toxin last for 3-6 months.  Duration of effect can be influenced by metabolism, excessive exercise, certain medications, sun exposure, smoking, and genetics.  If treatments are repeated regularly, just as the muscle action starts to return, the treated muscle will get weaker and weaker over time, leading to the effect of Botulinum Toxin lasting longer.  For most patients, a Botulinum Toxin treatment every 4 months is optimal.

Regular and long-term use of Botulinum Toxin helps to prevent lines and wrinkles from becoming deep and ingrained.  For this reason,  it is regarded as a preventative treatment.

Botulinum Toxin for masseter hypertrophy (to decrease jaw clenching and teeth grinding) and underarm hyperhidrosis often lasts for up to 8 months.

Fillers

What are they?

Soft tissue fillers are substances injected into the skin to create a more youthful appearance.  Many fillers are available but the most popular fillers are made from hyaluronic acid, a clear gel that is hydrophylic, allowing it to correct wrinkles and volume deficits by drawing in water.   Hyaluronic acid is a substance which naturally occurs in our skins, keeping it firm and hydrated.  Fillers made from hyaluronic acid are very similar in structure but are cross-linked to allow them to last for longer in tissue before our bodies break them down.

Different thickness gels (fillers) are used in different areas, according to the condition being treated. Fillers have become more and more popular and are the second most requested treatments in world. Results are soft, natural and long-lasting.

What does a treatment entail?

The area to be injected will be cleaned and the filler will be injected by your doctor.  One or more fillers may be injected according to the treatment plan agreed to by you and your doctor. Some patients only require one filler, others may need multiple fillers to achieve best results.  Often, patients choose to undergo the process in a step by step fashion over time.

Is there pain and downtime?

Ice is applied to the area being treated which helps make the injection less painful.  Most people tolerate the procedure well.  If you are particularly scared of needles, please ensure that you arrive at least 30 minutes before your appointment and request topical anaesthetic cream to be applied.  You may experience mild tenderness in the area for a few days afterwards, and bruising may occur.  Other rare side effects are covered in your consent form and can be discussed with your doctor.

When can I expect to see results?

Results are immediate but improve over the next 2-4 weeks as the filler draws in water and integrates with your tissue.  The result is soft and natural-looking, with no loss of movement in the treated area.

Biostimulators

What are they?

Biostimulators are compounds that are injected into the skin to stimulate new collagen formation. The aesthetic industry has been flooded with these new treatments which offer us a very exciting way to treat patients looking for an excellent rejuvenating skin treatment. Unlike fillers which restore volume only, biostimulators are able to restore some volume but their biggest advantage is their collagen stimulating effects which occur over a few months and lead to a lifting and tightening effect on tissue.

What’s exciting is that biostimulators are helping to treat patients where fillers aren’t always the best option. In those who have severe volume depletion, particularly in the lower face, fillers can sometimes be counterproductive, pulling tissue downwards instead of lifting it.

There is an immediate lifting effect when biostimulators are injected but the real magic occurs over the next few months as further lifting and tightening of tissue occurs, with an improvement in skin quality.

There are various products on the market, including a hybrid product containing a combination of filler and biostimulator. Your doctor is able to advise on which one would be best for you.

What does the treatment entail?

Pre-treatment photographs will be taken. Your skin will be disinfected and treatment markings made. The biostimulator is then injected into the correct layer of the skin using a cannula which allows us to disperse the product evenly. Afterwards, the area will be massaged.

Is there pain and down time?

You may feel quite tender and mildly swollen for a few days after a treatment. Oral analgesics are rarely needed but paracetamol may be taken if required. As with any injectable, bruising is possible. Avoid anti-inflammatories for two weeks after a treatment as these may hamper collagen production and lead to a diminished result.

When can I expect to see results?

There is likely to be some immediate result due to volumisation, depending on the choice of biostimulator. Collagen takes time to form and mature and results can be expected to improve over the next 6-9 months.

How many treatments do I need and how often?

For best results, more than one treatment may be needed and is dependent on age and quality of skin of patient as well as product used. Your doctor will advise on what is best for you as an individual.

Liquid Facelift

What is it?

A liquid facelift is the name given to an overall rejuvenation of the entire face using only Botulinum Toxin and soft tissue fillers.  Using a combination of Botulinum Toxin and multiple fillers, it is possible to make a patient look so much younger that it almost looks as though they’ve had a real facelift!

A liquid facelift is a non-invasive alternative to a surgical facelift.

Who would benefit from it?

Any patient who feels that they have aged and their facial skin is sagging, would benefit from a liquid facelift.  Some patients have medical conditions that preclude them from having a surgical facelift.  Many others are unwilling to undergo a general anaesthetic or an invasive procedure.  Others would prefer to delay having plastic surgery for a few years.  All of these patients are good candidates for a liquid facelift.

As we age, our skins lose fat, collagen, elastin, and hyaluronic acid.  Bone and fat pads gradually become resorbed. Our faces seem to deflate like a balloon, with the development of sagging, wrinkles and creases, and fine lines.  Years of frowning, smiling, and lifting the eyebrows leads to excessive wrinkles.

Botulinum Toxin is used to treat lines and wrinkles caused by overactive muscles (usually frown lines, forehead creases, and crow’s feet ) . Fillers are used to restore volume and to correct wrinkles.  Thicker fillers are used to replace volume in areas which start to sag or which decrease in size due to bone or fat resorption.  They can also be used to strengthen jawlines, restore chins, fill in hollow temples, and correct nose defects.  Medium thickness fillers are used to lift lines and creases, fill in hollows, and create contours.  Very thin fillers are used to fill small lines such as those around the eyes and lips.  Skin boosters may be used to help restore the quality of aging skin.

A  combination of Botulinum Toxin and different types of fillers is used to give optimal results.  Your doctor will be able to advise what kind of a result you could expect from your liquid facelift.

What does a treatment entail?

You will be photographed from various angles and your doctor will use these photographs to determine exactly what is needed to achieve a liquid facelift.  This will be discussed with you and a quotation given.  You may choose to undergo your liquid facelift in one session, or in multiple sessions. A treatment plan will be drawn up.

The liquid facelift involves multiple injections of Botulinum Toxin and soft tissue fillers.  Sometimes a cannula is also used to achieve widespread placement of the filler.  Ice is used to numb each area prior to injecting.

Is there pain and downtime?

Pain is minimal and most patients tolerate the procedure well.  The use of ice to help numb the area is usually sufficient.  If you are particularly scared of needles, please arrive 30min before your treatment time and ask for a topical anaesthetic to be applied.

Downtime is minimal, but as with any injection, bruising may occur.  You may experience mild tenderness in the injected areas for a few days afterwards.  Other rare side effects of Botulinum Toxin and fillers are covered in your consent form and can be discussed with your doctor.

When can I expect to see results?

Botulinum Toxin takes two weeks to reach maximum effect.  Results from fillers are immediate but improve over the next 2-4 weeks as they draw in water and integrate with your tissue.  The overall result from a liquid facelift is usually a more youthful, rested and natural looking appearance without any scars.

Skin Boosters

What are they?

Skin boosters (skin quality injectables) are a specific type of filler that is injected very superficially in the skin to improve the quality of the skin.  Skin boosters contain hyaluronic acid which has a large affinity for water.  Multiple small droplets of this are injected into the skin, drawing in water and improving hydration and elasticity of the skin, and helping to reduce the appearance of fine lines and wrinkles.  The result is a smoother, more hydrated, and more youthful appearance of the overlying skin.  Skin boosters also help to stimulate collagen production in the skin leading to an overall improvement in skin quality.

Where can skin boosters be used?

Skin boosters can be used wherever improved hydration and elasticity of the skin is desired.  They are ideal for treating crepiness and fine lines and wrinkles, especially around the eyes, lips and the neck and décolletage. They can also be used to treat tiny defects such as acne scars.  Although skin boosters are a type of filler, they are not used to treat deep lines and wrinkles or to restore volume, but to improve the overall quality of skin.

The area to be injected will be cleaned and a topical anaesthetic applied for 30-45 minutes.   Multiple small injections of the skin booster are placed just under the skin using a tiny needle.  The treated area is then massaged to ensure widespread distribution of the product.  Small bumps may be felt in some places for 2-3 days.

Is there pain and downtime?

The use of a topical anaesthetic means that the procedure is almost painless.  Often, small bruises will occur which disappear in a few days, and can usually be covered with makeup.

Skin boosters can also be deposited under the skin using a cannula which does not require a topical anaesthetic and usually results in no bruising at all.

Your doctor will discuss both options and the pro’s and con’s of each method of treatment with you.

When can I expect to see results?

Results are usually evident within two weeks of a treatment.  You can expect an improvement in hydration and elasticity and overall skin quality in the treated area.  There is very little volumising effect and the result is soft and natural-looking, with no loss of movement or sensation in the treated area.

A PRP facelift is the latest facial rejuvenation treatment  which is gaining a huge following  in patients looking for a completely natural rejuvenation procedure.  Platelet-rich plasma (PRP) derived from the patient is injected into areas in the face that require rejuvenation and volume restoration.  Typical areas include the cheeks, under-eye hollows, nasolabial lines, lips, and chin area.

PRP contains a concentrated amount of growth factors and stem cells that stimulate tissue rejuvenation and collagen formation.  Whilst results are not as marked as those obtained using injectable fillers, many patients prefer this completely natural process of facial rejuvenation.

What does a treatment entail?

A few small tubes of blood are taken from your arm and placed into a centrifuser which spins it down, separating the red blood cells from your plasma. This plasma is rich in stem cells and platelet-derived growth factors which help to stimulate maximal collagen production.  We use your plasma as an injectable “filler”, injecting it into areas that require rejuvenation and volume restoration.

Is there pain and downtime?

As with any injection, there is a small amount of discomfort and bruising may occur.  Discomfort is minimalised with the use of ice beforehand to numb the area to be injected.  Aftrerwards, the injected areas are likely to be slightly red and tender for a few hours due to the resulting increase in blood flow and stimulation of active rejuvenation processes taking place.  Bruising, if it occurs, will disappear in a few days.

What do I need to do afterwards?

No specific aftercare is necessary.  Please avoid taking anti-inflammatories for a few days after your treatment as this may hinder the rejuvenation process.

When can I expect to see results?

Most people see results within a few weeks after treatment which can improve for up to three months.

How many treatments do I need and how often?

We recommend at least 3 treatments at 1 monthly intervals for optimal results.
